Who qualifies for a yellow card?
Residents
People moving to Denmark for over 3 months (some EU/EEA cases 6 months) with a registered address.

How to apply (step-by-step)
1) Get CPR
Register your address at the kommune or an ICS center and obtain a CPR number.
2) Apply online
Submit the health card application via borger.dk.
Alternative: in person
If online isn’t possible, go to your kommune’s Borgerservice with passport/ID, permit, and address proof.

Select a GP
Pick an available GP during application. Check availability on sundhed.dk.
Change later
You can change GP for a fee (about 220 DKK in 2025), free if you move to a new area.
